‘I Wasn’t Able to Even Open My Eyes. That’s How Swollen I Was’

Onemanda Preisinger experiences worry about her child’s impending 13th birthday party. It’s not due to the typical reasons associated with a house full of loud preteen kids, but because it’s the first time she will reveal her recently altered face to friends and relatives. “Clearly I’m going to inform all guests as they come in, ‘For your information, this is not the way I appear,’” states the 30-year-old property agent from south Florida.

Her appearance is, well, a little surprising – her face puffed up and unnaturally tightened, as though held together by heavy-duty tape. Her new – and she’s eager to emphasize, short-term – look is the outcome of half a dozen cosmetic procedures, including an endoscopic mid-facelift, performed by a surgeon in Istanbul, Turkey, the previous month. “My poor husband became emotional when he saw me for the initial time because I wasn’t able to even unseal my eyes. That’s how puffy I was,” she explains to me via video call from her home. “I needed to inform him: ‘Honey, I’m okay, I’m not in pain. I just look like someone attacked me.’”

Increasing Popularity of Youthful Facial Lifts

According to cosmetic specialists, Preisinger is one of a growing number of individuals choosing to undergo a facelift in their 20s and 30s – well over a decade before most doctors’ usual candidate age of forty to sixty. (Although many specialists are eager to highlight the industry edict of: “We treat genetics, not years.”) “I have 28-year-olds asking for facelifts,” states based in London aesthetic plastic surgeon Georgios Orfaniotis, a former NHS consultant who focuses on the head and neck. “It’s a very significant procedure, and I’m a somewhat worried when I see people opting for it as a personal preference.”

Explaining the Rhytidectomy Surgery

A facelift, also known as a facelift, elevates the ligaments in the face that start to sag as we age. “Human faces are structured a little like onion layers,” explains Kent-based facial plastic surgeon Marc Pacifico. “Dermis and adipose on the surface, then a stratum of supportive tissue called the SMAS. Consider the facial framework as the soft tissue skeleton of the face. And that is what ages us; that’s what loosens and sags and pulls the dermis downward with it.”

You risk operating on individuals that have underlying problems. It’s not advisable for an ethical plastic surgeon to pursue

Previously reserved of affluent seniors, overuse of injectables – when excessive use of dermal fillers expands the face and causes looseness in the skin – alongside the common adoption of slimming medications, low-cost surgical travel, and the development of new, famous-figure-promoted operative methods are driving a different type of patient towards this major operation. Reports indicate an eight percent rise in facial lifts over the last year in the UK; while a survey revealed that the percentage of youthful candidates is growing, with 32% of facelifts now conducted for individuals aged thirty-five to fifty-five.

“People are asking now to appear as the best version of themselves and obviously the techniques are evolving,” comments Orfaniotis. Currently, the operation du jour is the comprehensive facial lift, a technique promoted by everyone from Kardashian matriarch Kris Jenner, to fashion designer Marc Jacobs.

Advanced Lifting Method

Where a more traditional rhytidectomy entails elevating the superficial layer, the deep plane facelift releases the underlying structures beneath it, enabling doctors to restructure the face by repositioning the underlying layers, and preventing the taut, pulled look occasionally caused by more traditional techniques. “We’re not putting stress on the dermis because we’re targeting deeper, to the deeper tissue,” explains prestigious London cosmetic doctor Rajiv Grover, who focuses on the advanced method. “It is the underlying layers that is adjusted. The skin follows along as a secondary element.” Lifting the entire soft tissue layer as a single unit allows for a authentic-appearing and more durable result. It also means that the procedure is no longer being used for the express purpose of anti-ageing – or rejuvenation, as the cosmetic field terms it – but for “beautification” and contouring, too. “We receive many inquiries from patients aged late twenties to mid-thirties for this reason,” notes Grover.

Individual Experience

Preisinger had never planned on undergoing a facelift – at least not at the thirty years old. But years of using injectable gels in an effort at “facial balancing” meant that by the time she reached her late twenties, she was “damaged”. “I didn’t undergo this because I wanted to look youthful,” she says. “I underwent it because the filler ruined my face. I regret ever using it. If I didn’t have the injectable, I don’t believe I would be where I’m at currently.”

Whether dermal fillers ever fully dissolve has long been a point of contention in the aesthetics industry. “Research have shown that not only do they rarely fully dissipate,” says Grover, “but they migrate and can block lymphatic channels. A contributing factor to what we call ‘puffy appearance’ is also the reality that to some extent, the face gets somewhat fluid-filled because its capacity to remove bodily fluid has been reduced.” Though studies into the area is early-stage, warnings on injectables’ possible effects on the lymphatic system have been released, and further research announced. One highly regarded face specialist, commenting anonymously, says he would avoid using fillers in a patient because of the risks they pose. “A lot of surgeons avoid discussing this, because the companies who make filler can be pretty aggressive.” He’s been told accounts, he says, of cosmetic doctors being landed with court orders after raising their concerns.

Decision and Procedure

For Preisinger, after beginning injecting filler, she felt as if she had to keep replenishing it – especially as it started to migrate to different areas of her face. She was only twenty-eight when a local plastic surgeon in the state proposed that a facial and neck surgery might be what she required to exit the cycle of treatments. After 24 months, she ended up selecting from a selection of suggested accommodations in the city, sent to her by the clinic assistant at her doctor’s clinic.

Initially, Preisinger had scheduled an upper and lower blepharoplasty – better known as eyelid surgery – but her doctor recommended that a facelift was the right solution for the looseness she was experiencing in her face. She arranged a brow lift, a cheek lift and a jawline surgery. The day before her surgery, one day after she’d arrived solo in Turkey, she decided to add a mouth enhancement. “Subsequently the surgeon said, ‘We’re going to lift the under-eye bags.’ Then he said, ‘I think if we remove some buccal fat, it will sculpt your face.’ Thus I ultimately including additional procedures,” she recalls. She paid $22,000 (sixteen thousand five hundred pounds) for the six-hour surgery, plus a three-day|
